A Panic Attack is provoked by an anxiety attack and/or anxiety, which is driven by stress, which is driven by worry about something. However, the panic attack seems to be provoked by nothing, or nothing of any significance. Panic attacks typically come suddenly, as if out of nowhere. They seem to strike at any time, often when driving in heavy traffic, in a crowded shopping mall, during a sleeping dream or in the middle of a business meeting. Clients tell me it is usually when they feel threatened by situations, they feel they cannot control.

My sister had been living in a small country town (of less than 500 people). She had to come to the city for a routine medical procedure. As she pulled up at the first urban traffic lights, after driving for 3 hours non-stop she had a panic attack. At first, she thought it was a heart attack, which only made her feel worse. An ambulance was called, and she was transported to hospital for monitoring. Tests confirmed it was only a panic attack, almost certainly triggered by the prospects of her medical procedure and the heavy traffic she encountered. She had her medical procedure without incident and went home. She has never had another panic attack. Once was enough.

Some people think they are experiencing panic attacks when they are only experiencing anxiety. When I ask these people to describe the symptoms of their attack, they only mention typical anxiety symptoms such as,

  • fatigue,
  • nausea,
  • back and chest pain,
  • clammy hands
  • shortness of breath,
  • trembling,
  • twitching,

While some people who have panic attacks only experience them occasionally, others experience them more frequently. Unlike anxiety symptoms, anxiety attacks and panic attack symptoms usually peak quickly. Thats why we call them attacks. Once it has passed sufferers usually feel fatigued.  Panic attacks typically include some or all of these symptoms:

  • Sense of impending doom,
  • Fear of death,
  • Rapid, pounding heart palpitations as if it could be a heart attack,
  • Body trembling in morbid fear,
  • Shortness of breath,
  • Tightness in the throat,
  • Chills and sweating,
  • Hot flashes,
  • Nausea,
  • Abdominal cramping,
  • Headache,
  • Dizziness, 
  • Numbness or tingling sensations,
  • Feeling detachment from reality.

Fear is an important factor in panic attacks. I think it is the emotion that makes it unique and different from anxiety. Once you have experienced a panic attack the fear of another only makes matters worse. The fearful anticipation of another panic attack, particularly as various triggers are recognized can, in some people, trigger another.

Physically, the body prepares to deal with a perceived external threat, which is most often irrational. Blood pressure and heart rate are increased, sweating is increased, blood flow to the major muscle groups is increased, and the immune and digestive system functions are inhibited for the fight or flight response. An anxiety attack is a usually brief feeling of intense anxiety. External signs may include pale skin, sweating, trembling, and pupillary dilation.

A person experiencing a panic attack will often feel very real dread or panic as if he or she is about to die or pass out. Although panic attacks feel dangerous, even deadly to the sufferer, they are not in themselves dangerous to the sufferer. The problem is the danger a panic attack can put the sufferer in as they are experiencing one.

I had a commercial airline pilot come to me for help with his panic attacks. He told me how he had often experienced them while he was piloting a plane full of paying passengers. he had always managed to ‘get through it’ before landing procedures began. Yet his greatest fear was having a panic attack while landing an aircraft full of passengers.  Then it happened. Perhaps because he focused on his fear so much, he created it? It was a rough landing which damaged the undercarriage of the aircraft. There was an investigation, and he was grounded. Only then did he decide to come to see me. I could have treated the symptoms of his panic attacks and sent him on his way, but that would have been unprofessional of me. Instead, I talked to him further about what had happened prior to his first ever panic attack. He told me how he had been standing on a street corner one warm evening when two vehicles collided in front of him. In the wreckage two people died. As a result, he suffered on going trauma, which was not treated.  While flying he started having flashbacks to the vehicle carnage, which he did not report because he feared he would be grounded as a pilot pending successful treatment. He knew doctors could only offer him medication.

Psychologists and counsellors would drag him through months of expensive treatment without any targetable results, all the while he would be grounded unable to fly. His career would effectively be over. So, he chose to keep it a secret, keep flying and hope for the best. It was only after he was grounded, and friends learnt the truth that some referred him to me. I treated his trauma around the fatal traffic accident and his crash landing, then his fears, anxieties about possible panic attacks and then panic attacks themselves. In that way he could be psychologically accessed as able to fly again, and free of anxiety, fear and panic attacks.

Panic attacks are more intense and that’s how they can be confused with heart attacks. Many who experience panic attacks feel trapped and unable to free themselves. Every anxiety sufferer does not experience panic attacks. They usually come without warning, and although the fear is generally irrational, the perception of danger is very real.

Panic attacks are commonly linked to agoraphobia and the fear of not being able to escape a bad situation. Within an anxiety attack, people may feel fearful and apprehensive. They may feel their heart racing or feel short of breath, but it’s very short-lived, and when the stressor goes away, so does the anxiety attack.
